Mail-In Repairs

  1. Fill out the form below. You may also download the CellTeks RA Form, fill it out and e-mail or fax it to us. We will review your repair request and issue you an RA #.
  2. Once you have received your RA #, you will need to mail in your phone with a copy of the RA form AND write the RA # on the outside of the box.
  3. CellTeks will contact you regarding your repair quote and payment arrangements. Your phone will be repaired and shipped back to you. Turnaround time is usually within 48 hours of receipt of your phone!

Please describe the nature of the problem with your phone. Please be specific and detailed in your description. This will aid us in analyzing your problem. Indicate all physical damage, liquid damage and/or software problems.

CellTeks cannot guarantee or warranty repairs involving liquid damage.
